Create a collection form
Go to Forms, create a form from a template, then set its title, what you accept (text, video), the fields you ask for and what happens after submit.
On this page
To create a collection form, open Forms, start from a template, then adjust the title, what you accept, the fields and the thank-you message. Your customers fill in this form to leave a testimonial.
Create the form
- Click Forms in the top menu.
- Create a new form and pick a template, for example SaaS and software, Coaches and consultants, Agencies, Freelancers or Blank. Templates come with a ready-made title and questions for your type of business.
Set the basics
Under Basics, set:
- Form title: the question your customer sees first, for example “How has Acme changed your work?”
- Intro: a short line under the title.
- Public slug: the end of your form link, for example
testimonials.ltd/c/acme.
Choose what you accept
Under What you accept, turn on Accept text testimonials, Accept video testimonials, or both. Video uses a slot from your account’s video slot pool. See Collect video testimonials.
Pick the fields
Under Fields, choose what you ask for besides the testimonial: name, title, company, photo and a social link. For each field you can change the Label and the Placeholder, move it up or down, or click Remove field.
Set what happens after submit
Under After submit:
- Thank-you message: shown after your customer sends the testimonial.
- Redirect URL: optional. Send people to a page on your site. If you point it at your Google or Facebook review page, the thank-you page also shows a button to post the review there too.
- Form is active: turn it off to stop accepting new testimonials.
